Personalized Note Pad Order Form

Note pads are printed in 2 colors, 3.5" X 8.5", 25 sheets per pad and printed on 20# white bond paper.

1) Email us your photo and logo, (we may already have your logo on file).

2) We accept Visa, Mastercard, Discover & AMEX by phone or fax and checks by mail.

3) Delivery Time: 2 days to 2 weeks after we have your approval to print.

*We will email you a pre-press proof for you to review and make edits to.

*There will be no refunds after an order is printed and the payment is processed. If we make a mistake on your order, we will gladly correct it and re-print the order. All orders placed are final and cancellation after artwork begins is $35.
